Diabetes Self Management Program

(Includes initial interview, 3-4 1 hour sessions utilizing diabetes flip chart, personal goals, and follow-up interview)

Session 1
1. Initial Diabetes interview - $40.00

2. Survival Skills - $30.00
- patient may purchase a combination of survival skills and BG monitoring sessions
- includes a follow-up phone call one week later to check progress and assess need for additional sessions

3. Blood Glucose Monitors - $10.00
- this includes instruction on how to use an individual BG monitor and assess patient technique
- includes follow up phone call one week later to check progress and assess for any need for additional sessions
- patient may bring in his/her BG readings after one week for pharmacist to evaluate

Session 2
1. Insulin Therapy - $25.00
- includes instruction on how to use, store, mix, and inject insulin as well as assessment of patient technique
- includes a follow-up call in one week to check progress and assess need for additional sessions

2. Oral Medications to Treat Diabetes - $20.00

3. Diet and Exercise to Treat Diabetes - $25.00
- includes a follow-up call in one week to check progress and assess need for additional sessions

Session 3
1. Ongoing Monitoring - $20.00
- includes instruction on how often to monitor parameters
- we will offer to type up any BG readings they take at home and any BP readings they take at home or at the store and send a copy to their physician

2. Avoiding the Long Term Complications of Diabetes - $20.00


Cholesterol Program

Session 1 (initial interview)
- Pathophysiology
- Patient medical history
- Risk factors and goals
- Introduction to exercise and diet
- Lipid panel if not done in the last 3 months

Session 2 (to be completed at week 4)
- Exercise and diet
- Patient education about medications

Session 3 (3rd Month)
- Review of exercise and diet
- Medication side effects and compliance

Session 4 (6th Month)
- Review of exercise, diet, and medication

CPCN Asthma Self Management Program

(includes initial interview, 3 1-hour sessions utilization asthma flip chart, personal goals, and follow-up)

Session 1
1. Survival Skills - $40.00
- This session uses the first section of the flip chart. The session includes a complete patient asthma history, an overview of asthma, discusses categories of asthma based on severity, and goals of therapy. The pharmacist also will help the patient get started on a list of possible triggers that therapy can add to over the next couple of weeks as they become increasingly aware of what is exacerbating their asthma.

Session 2
1. Medications Used in the Treatment of Asthma - $35.00
- This session uses the first part of section 2 of the flip chart. Further assessment of symptoms, progress and goals of therapy, and triggers.

2. Proper Use of Inhalers and Accessory Devices - $10.00
- This session uses the last part of section two of the flip chart. Patient's technique will be evaluated by the pharmacist. The patient has to demonstrate correct inhaler technique before the teaching session is considered complete.

3. Administering Medications through a Nebulizer - $10.00
- The pharmacist will discuss the parts of the machine, how the machine works, hooking up the tube to the machine and mask to the tubing, putting medication in the proper container, cleansing of the nebulizer and all parts, explanation of all medications to be used with the machine, and storage of the machine and all accessory parts.

Session 3
1. Ongoing Monitoring and Management of Asthma - $35.00
- The pharmacist will demonstrate the use of a peak flow meter. A log chart will be sent home with the patient for them to record their values. Further assessment of symptoms, progress and goals, triggers, and pharmacotherapy.

2. Individualized Patient Care Plan
- This will provide the patient with an action plan for their medications based on their peak flow readings and emphasize patient understanding of how to use their action plan. Will also reassess how patients will continue to reduce/avoid triggers and asthma symptoms.
